Got in the mail a letter from Bank of America asking for a substitute w-9 to be filled out immediately or my account would be closed. Funny thing was, there was no reference to the specific account OR a phone number to call if I had a question. So I pull out my card and call Bank of America. After about 30 minutes of dealing with a very nice gal who also put me in touch with fraud, the information and copies of the documents are now in their hands.

BUT wondering how many people fill these things out and send them in - especially with that line about your account being closed.
